Cause of death: Demolished to make way for four new townhomes at 2904-2910 Park Avenue. The townhome project was first proposed in 2006, but stalled. The building then changed ownership in September after the previous owner declared bankruptcy following the City's pursuit of criminal prosecution for code violations.
